Requesting your medical records and/or clinical documentation for FMLA, disability, or other leaves

Can my Seven Starling providers complete paperwork or provide letters for FMLA, disability, or other leaves?

Please note that Seven Starling does not provide assessments or participate in the completion of paperwork/forms relating to leaves of absences, disability claims, etc. Please see our FAQ for more information.

It's important to note that these types of forms may require specialized assessments, physical examinations, or oversight that extend beyond the scope of care provided by Seven Starling. The best person to assist is typically your PCP, OBGYN, or another provider who manages your comprehensive care.

We recommend sharing those records with your PCP, OBGYN, or another provider managing your comprehensive care, and asking them to complete any needed paperwork.

How can I request my records?

To have your medical records from Seven Starling sent to you or to another provider, email us at support@sevenstarling.com. We’ll ask you to complete an authorization form which will give us your permission to share your records with you directly or another provider.
